A tragic head-on collision on the N4 highway near the Middelburg Dam in Mpumalanga has claimed the lives of three people and left five others injured. The devastating crash, which occurred on Friday, 28 November, once again highlights the dangers motorists face on one of South Africa’s busiest national routes.
According to Mpumalanga Department of Community Safety, Security and Liaison spokesperson Moeti Mmusi, the accident involved two bakkies travelling in opposite directions. Preliminary reports indicate that one of the vehicles, which was heading from Mbombela, lost control and veered into the oncoming lane. This sudden lane change resulted in a violent head-on collision with another bakkie travelling from Emalahleni.
The impact was catastrophic.
Emergency responders arrived to find widespread wreckage and several passengers trapped in the vehicles. Authorities confirmed that three occupants — the male driver, a female passenger, and a young boy — died at the scene due to the force of the crash. Their identities have not yet been released, as police work to notify the families.
Five other people survived but were rushed to a nearby hospital for urgent treatment. Among them were two young girls who sustained serious injuries and required immediate medical attention. The second bakkie’s driver and two additional passengers — a woman and a young boy — suffered minor injuries but were transported for precautionary care.
Traffic on the N4 was heavily affected as authorities worked to clear the scene and investigate the circumstances leading to the collision. Motorists were diverted while police conducted an on-site assessment and emergency personnel removed debris from the roadway.
Mmusi confirmed that a full investigation is underway to determine what led to the loss of control that sparked the deadly crash. While mechanical failure, fatigue, speeding, or unsafe overtaking have not been ruled out, investigators will rely on witness accounts, vehicle assessments, and environmental factors to establish the exact cause.
The N4 remains one of the most vital yet dangerous routes in Mpumalanga, carrying high volumes of traffic daily, including heavy-duty vehicles and long-distance travellers. The latest incident has sparked renewed concern about road safety, especially with the festive season approaching — a period historically marked by increased crashes and fatalities.
