The family of Grade R learner Manqoba Mnisi is searching for answers following his tragic death, allegedly after he was locked inside a classroom at Bernard Isaacs Primary School in Coronationville, Gauteng.
Gauteng MEC for Education, Matome Chiloane, has appointed an independent law firm to investigate the circumstances surrounding the young boy’s passing. The firm, Buthelezi-Vilakazi Incorporated, has been given two weeks to conduct a full and impartial probe into the incident.

Manqoba, who was from Jabulani in Soweto, reportedly sustained injuries on the school premises on Monday, 2 February 2026. He was later taken to hospital, where he sadly passed away. Details surrounding exactly how he was injured remain unclear, prompting public concern and calls for accountability.
According to the Gauteng Department of Education, inconsistencies and unanswered questions about the incident led to the decision to appoint an independent investigative team. The department said it was important to ensure transparency and restore confidence among the family and the broader school community.

Departmental spokesperson Steve Mabona confirmed that MEC Chiloane first met privately with the Mnisi family to formally introduce the appointed law firm and explain the scope of the investigation. Chiloane later visited Bernard Isaacs Primary School in Coronationville, where he introduced the investigative team to the School Management Team as well as members of the School Governing Body. During these engagements, he outlined the terms of reference and the objectives of the investigation.
The law firm has been mandated to conduct comprehensive interviews with all individuals who may have information relevant to the case. This includes the district director, circuit manager, cluster leader, school principal, educators, general assistant, security personnel, learners, community members, and Manqoba’s parents. The aim is to piece together a clear timeline of events and determine whether any negligence or misconduct occurred.
